第一章： 聖經的命令
The Bible Mandate

1.1 We confess that the Bible teaches concerning repentance unto life, “Men ought not to content themselves with a general repentance, but it is every man's duty to endeavor to repent of his particular sins particularly [Psalm 19:13, Luke 19:8, 1 Timothy 1:13, 1 Timothy 1:15].” And “As every man is bound to make private confession of his sins to God, praying for the pardon thereof, [Psalm 32:5-6, Psalm 51:4-5, Psalm 51:7, Psalm 51:9, Psalm 51:14] upon which, and the forsaking of them, he shall find mercy; [Proverbs 28:13, 1 John 1:9] so he that scandalizeth his brother, or the Church of Christ, ought to be willing, by a private or public confession and sorrow for his sin, to declare his repentance to those that are offended, [Psalm 51, Joshua 7:19, Luke 17:3-4, James 5:16] who are thereupon to be reconciled to him, and in love to receive him [2 Corinthians 2:8].” (Westminster Confession of Faith 15:5-6).

1.2 Therefore, we believe that cases of sexual abuse, as a public scandal against both the magistracy and ministry over which Christ Mediatorially reigns, requires all offenders to publicly repent to both God, and man in both spheres of authority. The truly repentant must repent to the Church of Christ through her courts, and also through the courts of the civil magistrate.

1.3 This truth has the corollary effect of likewise requiring church officers to have a child protection plan in place, and in the event that knowledge of abuse occurs, to report all cases of sexual abuse to the civil magistrate. This fulfills the requirement of the sixth commandment to preserve life “by just defence thereof against violence” [Psalm 82:4; Proverbs 24:11-12; 1 Samuel 14:45] and “protecting and defending the innocent [1 Thess. 5:14, Job 31:19-20; Matt. 25:35-36; Proverbs 31:8-9].” (WLC 135). The fact that Christ requires reporting to the civil magistrate is particularly clear since sexual abuse is universally recognized as public sin, as witnessed by the actions of the civil magistrates in both the United States of America and Canada that keeps public national registries of convicted sexual offenders for the good and safety of the public.

1.4-Further observations to strengthen our resolve to fulfill this Bible Mandate is in the following biblical precepts:

Be not ye therefore partakers with them” (Ephesians 5:3,6-7).

1.4.3-We are to be rebuking and exposing the darkness conducting ourselves wisely and walking circumspectly: “And have no fellowship with the unfruitful works of darkness, but rather reprove them. For it is a shame even to speak of those things which are done of them in secret. But all things that are reproved are made manifest by the light: for whatsoever doth make manifest is light. Wherefore he saith, Awake thou that sleepest, and arise from the dead, and Christ shall give thee light. See then that ye walk circumspectly, not as fools, but as wise, Redeeming the time, because the days are evil. Wherefore be ye not unwise but understanding what the will of the Lord is” (Ephesians 5:11-17).

Finally, we know that we are surrounded by wolves and enemies of God on every side, and need to be wise: “Behold, I send you forth as sheep in the midst of wolves: be ye therefore wise as serpents, and harmless as doves” (Matthew 10:16).

Sessions are directed to schedule this Plan to be presented to the congregation by the Pastor, or the Diaconate Chair, at every annual meeting of the Congregation, for the purpose of keeping the Plan and her policies fresh in the minds of members.

The Presbytery requires all staff members and volunteers working with any child under the age of 18 to complete four safety steps before ministry work or volunteer placement can begin.

All officers, members, and volunteers working with minors must undergo and pass a criminal background check. Some congregations in the Presbytery may also require the staff member or volunteer to obtain a fingerprint-based clearance certificate or card as part of the criminal background check. The criminal background check will include a search of the Sex Offender Registry; this includes a search of the Registry for any minors who wish to volunteer by working with under minors and are at least 14 years of age. The background check will be renewed every year.

Officers, members, and volunteers are required to review the policies contained in this Plan and sign the Statement of Understanding and Agreement (attached to this Plan) indicating that he or she has read and understood the material and agrees to comply with the Plan’s requirements.

Officers, members, and volunteers need to have a basic understanding of the characteristics of sexual abusers and their behaviors in “grooming” a child for sexual abuse. “Grooming” is the process used by an abuser to select a child, win the child’s trust (and the trust of the child’s parent or guardian), manipulate the child into sexual activity, and keep the child from disclosing abuse. All officers, members, and volunteers are required to complete a sexual abuse awareness training before working with minors. This training must be renewed once every two years and will be offered at annual meetings.

Final approval must be authorized by the Session before a person is authorized to work with children. Known convicted child abusers and/or sex offenders will never be allowed to participate in any ministry involving minors.

Reporting Suspicious Behaviors, Suspected Abuse and Accusations

The Presbytery has a zero-tolerance policy for known or suspected child abuse.

In the event that any staff member or volunteer observes any inappropriate behavior (e.g., violations of this Plan, neglectful supervision, grooming, poor role-modeling, etc.) or suspected abuse or neglect of a minor (physical, emotional, or sexual), it is that individual’s responsibility to immediately report these observations to the elders, or if the conduct involves the elders, to the Presbytery.

Because sexual abusers groom children for abuse, it is possible an officer, member, or volunteer may witness behavior that does not rise to the level of abuse but instead appears to be grooming behavior. Therefore, any inappropriate or suspicious behaviors and accusations of suspected child abuse must be reported to the elders using the Incident Report Form (attached to this Plan).

When physical or sexual abuse is reasonably suspected, it must be taken seriously and immediately reported to law enforcement and/or any other appropriate state agency in accordance with this Plan and state law. This is true regardless of whether the suspected abuse occurred on church property or involved church members. In other words, abuse of a child will be reported even if the perpetrator is not a church attender and the abuse took place off of church grounds. A summary of the relevant state law is outlined in Section 10.

If the report to law enforcement or the appropriate state agency is made in writing, a copy of that report will be maintained in Session records along with the Incident Report Form.

Once physical or sexual abuse of a child is reasonably suspected, further investigation of the abuse will be handed over to law enforcement which utilizes trained forensic interviewers to communicate with children and implements law enforcement tactics to interview the alleged perpetrator and any possible witnesses. The Session has an obligation to conduct its own ecclesiastical processes according to the Constitution of the Reformed Presbyterian Church of North America. However, the Session recognizes that a separate investigation conducted by the church could undermine a future criminal case, particularly if the church conducts its own interviews of victims and witnesses. Therefore, the church will not conduct an interview of any minor who has been the victim of physical or sexual abuse once the abuse is known and reported to law enforcement. The church will instead seek information about what occurred from the minor victim’s parents or guardians. The parent or guardian may choose to provide the church with a copy of any interview law enforcement conducted of the minor victim. Similarly, the church should attempt to obtain records of law enforcement interviews of witnesses or written statements by witnesses. If the church is not able to obtain these records, the church may conduct its own interviews of witnesses. All witnesses who are church officers, members, or volunteers must document what occurred using the Incident Report Form while the event is still fresh in their minds.

Anyone reasonably suspected of child abuse will be suspended from serving in any ministry to minors pending the outcome of the criminal investigation. (See Section 9.)

Monitoring and Supervision of Ministry Events

Known convicted child abusers and/or sex offenders will never be left alone with minors on church premises or during any church activities.

The Diaconate will design and implement sign-in/sign-out sheets for all children’s ministry activities for children aged 12 or younger.

No child aged 12 or younger will be left unattended or unsupervised during children’s ministry events.

Parents have an open invitation to observe all programs and activities in which their child is involved. However, parents who desire to participate in or have continuous, ongoing contact with their child’s program will be required to complete the church’s volunteer application and screening process. (See Section 2 above.)

If a minor goes missing during church events or on church property, and that child cannot be found within 10 minutes, a search team will be assembled from adults on hand to search the area (internal and external spaces) for the missing child.

If a minor becomes ill or sustains a serious injury, an officer, member or volunteer will contact the minor’s parents or guardians and have them come retrieve the child or provide guidance on how to proceed. If the minor’s illness or injury is so severe that a hospital visit is necessary, the officer, member or volunteer will call 9-1-1 or transport the minor to the hospital himself or herself. Transporting a minor to a hospital to receive treatment for a serious illness or injury is the only time an adult may transport a minor alone. In all other circumstances, officers, members, and volunteers must follow the Transportation policy discussed below.

Officers, members, and volunteers are prohibited from possessing or using illegal drugs. Officers, members, and volunteers are prohibited from possessing or using alcohol or tobacco while on church property, during ministry events, or in front of minors. The only exception is for the sacramental wine for the Lord’s Supper. The moderate use of alcohol by adults during fellowship in members’ homes is also acceptable if not a stumbling block for others.

Occasionally, minors – and in particular teenagers – might require a one-on-one conversation with a trusted adult. In those circumstances, the officer, member, or volunteer taking the meeting with the minor will obtain prior approval from either the elders or the minor’s parents. The conversation will take place in a public space where the interactions can be easily observed and where other adults are present to view the interaction. Additionally, the minor must not be given the impression that the communication will be kept confidential. If the minor discloses abuse, it must be reported as further described in Section 3.

If a teenager is unruly or fails to comply with verbal warnings or instructions from officers/members/volunteers, that teenager will be asked to leave (if not endangered by doing so) or the student’s parent will be contacted to pick up the student. In the event of a fight or physical altercation, officers, members, and volunteers will first verbally redirect students involved and will try to avoid physical intervention. However, physical force may be used as necessary when reasonably necessary to prevent physical harm or injury to any person, including one’s self. Uncontrollable or unusual behavior will be reported immediately to the teenager’s parents and/or the elders.

Any officer, member, or volunteer who is arrested for breaking any laws related to child abuse must notify the church within 24 hours of arrest. This suspension will continue during any investigation by law enforcement or other appropriate state agency. Although it may be legally sufficient to report only to a child welfare services agency in some circumstances, the Presbytery suggests immediate reporting be made to both the appropriate law enforcement agency and child welfare services agency. However, if the abuse victim implicates a child welfare services or law enforcement agency in the abuse, the report should only be made to the non-offending agency. The Session should notify the minor victim’s parent(s)/ guardian(s) immediately after making this report unless the parent or guardian is the accused offender.

Other persons who must report child abuse or neglect when they learn about it in their professional or occupational capacities include: licensed or certified behavior analysts, emergency medical professionals, medical professionals, pharmacists, radiologists, physical therapists, occupational therapists, athletic trainers, massage therapists, music therapists, dietitians, mental health professionals, social workers, and counselors; any employee of a medical facility; coroners; school employees and volunteers; any employee of a child care facility or camp; a foster home license-holder; any law enforcement officer or probation officer; an attorney (except in limited circumstances where the attorney represents the accused offender or the victim); any employee or volunteer for an agency that advises people about child abuse and neglect; any employee or volunteer working at a youth shelter; and any employee of any entity that provides organized activities for children. Each such report shall contain: (1) the name, address, age and sex of the child; (2) the name and address of the child’s parents or other person responsible for the care of the child; (3) the nature and extent of the abuse or neglect of the child; (4) any evidence of previously known or suspected abuse or neglect of the child or the child’s siblings; (5) the name, address and relationship, if known, of the accused offender; and (6) any other information required by the agency receiving the report. Although an exception to the reporting requirement exists for members of the clergy who acquire knowledge of the abuse or neglect from the offender during a confession, Session clergy will not invoke this exception. Because the Presbytery believes sexual abuse is a public sin against this land, the priest-penitent privilege does not apply to any statements concerning sexual abuse that are made to any officers, members, or volunteers. Similarly, the reporting requirements under N.R.S. § 432B.220 only apply to clergy who learn about abuse while acting in a professional or occupational capacity. Nevertheless, it is the expectation of the Presbytery that its clergy will report child abuse even when they learn about it in their personal lives.

The Pacific Coast Presbytery (PCP) is committed to providing a safe and secure environment for children and their families. To this end, any report of inappropriate behaviors or suspicions of abuse will be taken seriously and will be reported in accordance with God’s Law, as applied through the Child Protection Plan. This form is to be used anytime an incident occurs during Pacific Coast Presbytery’s or her member congregations’ ministry events, whether on-site or off-site, to provide a written record of inappropriate or suspicious behavior towards minors (e.g., “grooming”), allegations of physical or sexual abuse of a minor, and any other Plan violations. In the event of a potentially serious incident, such as suspected grooming, Plan violations, or allegations of abuse, it is highly encouraged that if at least two eyewitnesses exist, they should each fill out their own Incident Report Forms.
When reports of suspected child abuse have been made to law enforcement or another appropriate state agency in writing, a copy of any written report should be maintained along with this Incident Report Form.

1.1 We confess that the Bible teaches concerning repentance unto life, “Men ought not to content themselves with a general repentance, but it is every man's duty to endeavor to repent of his particular sins particularly [Psalm 19:13, Luke 19:8, 1 Timothy 1:13, 1 Timothy 1:15].” And “As every man is bound to make private confession of his sins to God, praying for the pardon thereof, [Psalm 32:5-6, Psalm 51:4-5, Psalm 51:7, Psalm 51:9, Psalm 51:14] upon which, and the forsaking of them, he shall find mercy; [Proverbs 28:13, 1 John 1:9] so he that scandalizeth his brother, or the Church of Christ, ought to be willing, by a private or public confession and sorrow for his sin, to declare his repentance to those that are offended, [Psalm 51, Joshua 7:19, Luke 17:3-4, James 5:16] who are thereupon to be reconciled to him, and in love to receive him [2 Corinthians 2:8].” (Westminster Confession of Faith 15:5-6).

1.2 Therefore, we believe that cases of sexual abuse, as a public scandal against both the magistracy and ministry over which Christ Mediatorially reigns, requires all offenders to publicly repent to both God, and man in both spheres of authority. The truly repentant must repent to the Church of Christ through her courts, and also through the courts of the civil magistrate.

1.3 This truth has the corollary effect of likewise requiring church officers to have a child protection plan in place, and in the event that knowledge of abuse occurs, to report all cases of sexual abuse to the civil magistrate. This fulfills the requirement of the sixth commandment to preserve life “by just defence thereof against violence” [Psalm 82:4; Proverbs 24:11-12; 1 Samuel 14:45] and “protecting and defending the innocent [1 Thess. 5:14, Job 31:19-20; Matt. 25:35-36; Proverbs 31:8-9].” (WLC 135). The fact that Christ requires reporting to the civil magistrate is particularly clear since sexual abuse is universally recognized as public sin, as witnessed by the actions of the civil magistrates in both the United States of America and Canada that keeps public national registries of convicted sexual offenders for the good and safety of the public.

Be not ye therefore partakers with them” (Ephesians 5:3,6-7).

1.4.3-We are to be rebuking and exposing the darkness conducting ourselves wisely and walking circumspectly: “And have no fellowship with the unfruitful works of darkness, but rather reprove them. For it is a shame even to speak of those things which are done of them in secret. But all things that are reproved are made manifest by the light: for whatsoever doth make manifest is light. Wherefore he saith, Awake thou that sleepest, and arise from the dead, and Christ shall give thee light. See then that ye walk circumspectly, not as fools, but as wise, Redeeming the time, because the days are evil. Wherefore be ye not unwise but understanding what the will of the Lord is” (Ephesians 5:11-17).

All officers, members, and volunteers working with minors must undergo and pass a criminal background check. Some congregations in the Presbytery may also require the staff member or volunteer to obtain a fingerprint-based clearance certificate or card as part of the criminal background check. The criminal background check will include a search of the Sex Offender Registry; this includes a search of the Registry for any minors who wish to volunteer by working with under minors and are at least 14 years of age. The background check will be renewed every year.

Officers, members, and volunteers need to have a basic understanding of the characteristics of sexual abusers and their behaviors in “grooming” a child for sexual abuse. “Grooming” is the process used by an abuser to select a child, win the child’s trust (and the trust of the child’s parent or guardian), manipulate the child into sexual activity, and keep the child from disclosing abuse. All officers, members, and volunteers are required to complete a sexual abuse awareness training before working with minors. This training must be renewed once every two years and will be offered at annual meetings.

Final approval must be authorized by the Session before a person is authorized to work with children. Known convicted child abusers and/or sex offenders will never be allowed to participate in any ministry involving minors.

Reporting Suspicious Behaviors, Suspected Abuse and Accusations

In the event that any staff member or volunteer observes any inappropriate behavior (e.g., violations of this Plan, neglectful supervision, grooming, poor role-modeling, etc.) or suspected abuse or neglect of a minor (physical, emotional, or sexual), it is that individual’s responsibility to immediately report these observations to the elders, or if the conduct involves the elders, to the Presbytery.

Because sexual abusers groom children for abuse, it is possible an officer, member, or volunteer may witness behavior that does not rise to the level of abuse but instead appears to be grooming behavior. Therefore, any inappropriate or suspicious behaviors and accusations of suspected child abuse must be reported to the elders using the Incident Report Form (attached to this Plan).

When physical or sexual abuse is reasonably suspected, it must be taken seriously and immediately reported to law enforcement and/or any other appropriate state agency in accordance with this Plan and state law. This is true regardless of whether the suspected abuse occurred on church property or involved church members. In other words, abuse of a child will be reported even if the perpetrator is not a church attender and the abuse took place off of church grounds. A summary of the relevant state law is outlined in Section 10.

If the report to law enforcement or the appropriate state agency is made in writing, a copy of that report will be maintained in Session records along with the Incident Report Form.

Once physical or sexual abuse of a child is reasonably suspected, further investigation of the abuse will be handed over to law enforcement which utilizes trained forensic interviewers to communicate with children and implements law enforcement tactics to interview the alleged perpetrator and any possible witnesses. The Session has an obligation to conduct its own ecclesiastical processes according to the Constitution of the Reformed Presbyterian Church of North America. However, the Session recognizes that a separate investigation conducted by the church could undermine a future criminal case, particularly if the church conducts its own interviews of victims and witnesses. Therefore, the church will not conduct an interview of any minor who has been the victim of physical or sexual abuse once the abuse is known and reported to law enforcement. The church will instead seek information about what occurred from the minor victim’s parents or guardians. The parent or guardian may choose to provide the church with a copy of any interview law enforcement conducted of the minor victim. Similarly, the church should attempt to obtain records of law enforcement interviews of witnesses or written statements by witnesses. If the church is not able to obtain these records, the church may conduct its own interviews of witnesses. All witnesses who are church officers, members, or volunteers must document what occurred using the Incident Report Form while the event is still fresh in their minds.

If a minor becomes ill or sustains a serious injury, an officer, member or volunteer will contact the minor’s parents or guardians and have them come retrieve the child or provide guidance on how to proceed. If the minor’s illness or injury is so severe that a hospital visit is necessary, the officer, member or volunteer will call 9-1-1 or transport the minor to the hospital himself or herself. Transporting a minor to a hospital to receive treatment for a serious illness or injury is the only time an adult may transport a minor alone. In all other circumstances, officers, members, and volunteers must follow the Transportation policy discussed below.

Occasionally, minors – and in particular teenagers – might require a one-on-one conversation with a trusted adult. In those circumstances, the officer, member, or volunteer taking the meeting with the minor will obtain prior approval from either the elders or the minor’s parents. The conversation will take place in a public space where the interactions can be easily observed and where other adults are present to view the interaction. Additionally, the minor must not be given the impression that the communication will be kept confidential. If the minor discloses abuse, it must be reported as further described in Section 3.

If a teenager is unruly or fails to comply with verbal warnings or instructions from officers/members/volunteers, that teenager will be asked to leave (if not endangered by doing so) or the student’s parent will be contacted to pick up the student. In the event of a fight or physical altercation, officers, members, and volunteers will first verbally redirect students involved and will try to avoid physical intervention. However, physical force may be used as necessary when reasonably necessary to prevent physical harm or injury to any person, including one’s self. Uncontrollable or unusual behavior will be reported immediately to the teenager’s parents and/or the elders.

To this end, officers, members, and volunteers will not talk to minors in a way that is or could be construed by any reasonable observer as harsh, threatening, intimidating, shaming, derogatory, demeaning, or humiliating. In addition, officers, members, and volunteers are expected to refrain from swearing in the presence of minors.

Suspensions/ Prohibitions from Ministry to Minors

Any officer, member, or volunteer who is arrested for breaking any laws related to child abuse must notify the church within 24 hours of arrest. Any officer, member, or volunteer who is accused of abusing a child (whether arrested or not) or of any act considered by the church to be harmful to a child will immediately be suspended from serving in any ministry to minors. This suspension will continue during any investigation by law enforcement or other appropriate state agency. Any person who is convicted of a crime involving child abuse will be prohibited from any future participation in any ministry to minors.

Apart from the foregoing, any confirmed violation of the Plan by an officer or member is grounds for immediate dismissal or disciplinary action. As a matter of Nevada law (N.R.S. § 432B.220), if any Session clergy member or employee knows or has reasonable cause to believe that a child has been abused or neglected, a report must be made as soon as practicable, but no later than 24 hours after learning of the abuse or neglect. As a general rule, the report must be made either to a child welfare services agency, e.g., the Clark County Department of Family Services or the Washoe County Child Protective Services Division, or to a law enforcement agency. However, if the offender is a volunteer or employee at a childcare facility, institution or home, the report must be made to law enforcement.

Although it may be legally sufficient to report only to a child welfare services agency in some circumstances, the Presbytery suggests immediate reporting be made to both the appropriate law enforcement agency and child welfare services agency. However, if the abuse victim implicates a child welfare services or law enforcement agency in the abuse, the report should only be made to the non-offending agency. The Session should notify the minor victim’s parent(s)/ guardian(s) immediately after making this report unless the parent or guardian is the accused offender.

Other persons who must report child abuse or neglect when they learn about it in their professional or occupational capacities include: licensed or certified behavior analysts, emergency medical professionals, medical professionals, pharmacists, radiologists, physical therapists, occupational therapists, athletic trainers, massage therapists, music therapists, dietitians, mental health professionals, social workers, and counselors; any employee of a medical facility; coroners; school employees and volunteers; any employee of a child care facility or camp; a foster home license-holder; any law enforcement officer or probation officer; an attorney (except in limited circumstances where the attorney represents the accused offender or the victim); any employee or volunteer for an agency that advises people about child abuse and neglect; any employee or volunteer working at a youth shelter; and any employee of any entity that provides organized activities for children. See N.R.S. § 432B.220(4).

The types of child abuse or neglect that must be reported include physical or mental abuse of a nonaccidental nature, sexual abuse, sexual exploitation, and negligent treatment or maltreatment, including harmful behavior that is terrorizing, degrading, painful or emotionally traumatic, abandonment of a child, leaving a child without proper care, control or supervision, or failing to provide adequate subsistence, education, shelter, medical care or other care for the well-being of the child. See N.R.S. §§ 432B.020; 432B.140. Note that excessive corporal punishment may constitute abuse or neglect. See N.R.S. § 432B.150.

Each such report shall contain: (1) the name, address, age and sex of the child; (2) the name and address of the child’s parents or other person responsible for the care of the child; (3) the nature and extent of the abuse or neglect of the child; (4) any evidence of previously known or suspected abuse or neglect of the child or the child’s siblings; (5) the name, address and relationship, if known, of the accused offender; and (6) any other information required by the agency receiving the report. See N.R.S. § 432B.230. The report should be by telephone but may be made by another means of communication if doing so will be quick and reliable, e.g., some agencies provide the option of filing an online report.

Although an exception to the reporting requirement exists for members of the clergy who acquire knowledge of the abuse or neglect from the offender during a confession, Session clergy will not invoke this exception. Because the Presbytery believes sexual abuse is a public sin against this land, the priest-penitent privilege does not apply to any statements concerning sexual abuse that are made to any officers, members, or volunteers. Similarly, the reporting requirements under N.R.S. § 432B.220 only apply to clergy who learn about abuse while acting in a professional or occupational capacity. Nevertheless, it is the expectation of the Presbytery that its clergy will report child abuse even when they learn about it in their personal lives.

The failure to report child abuse as required by Nevada law is a misdemeanor crime punishable by up to 6 months in jail and a $1,000 fine. A second or subsequent violation of the mandatory reporting statute is a gross misdemeanor punishable by up to 364 days in jail and a $2,000 fine.

The Pacific Coast Presbytery (PCP) is committed to providing a safe and secure environment for children and their families. To this end, any report of inappropriate behaviors or suspicions of abuse will be taken seriously and will be reported in accordance with God’s Law, as applied through the Child Protection Plan. This form is to be used anytime an incident occurs during Pacific Coast Presbytery’s or her member congregations’ ministry events, whether on-site or off-site, to provide a written record of inappropriate or suspicious behavior towards minors (e.g., “grooming”), allegations of physical or sexual abuse of a minor, and any other Plan violations. In the event of a potentially serious incident, such as suspected grooming, Plan violations, or allegations of abuse, it is highly encouraged that if at least two eyewitnesses exist, they should each fill out their own Incident Report Forms.
When reports of suspected child abuse have been made to law enforcement or another appropriate state agency in writing, a copy of any written report should be maintained along with this Incident Report Form.
